An Analysis of 173 Surgical Biliary Tract Diseases.


Abstract
An analytical reviews of the clinical records of 173 patients who had been admitted to the department of generbi surgery of Jeonbug national university hospital for 6 years from June. 1976 to June. 1982, was made to get the following results.
1) Of the 173 patients, 150 patients(86.7%) were benign disease and 23 patients(13.3%) were malignant patients.
The average age was 49.8 years old.
2) The ratio of male and female was 1 to 1.4 in all patients, 1 to 1, 3 in benign patients, and 1 to 1 in malignant patients.
3) The seasonal distribution of the incidence was predominantly high in spring, esp. March, followed by summer and autumn.
4) The duration from the onset of the symptoms to admission was between 1 month to 1 year in 24.3%, and over 10 years in 10.4%.
5) The most frequently presenting symptom was abdominal pain(85.5%), and jaundice (18.6%), fever and chillness (14.8%) were next in order.
6) Laboratory findings were elevation of alkaline phosphatase (67.6%), SGOT, and total bilirnbin on liver function test, and leukocytosis(56%) and anemia(22.5%) on CBC.
7) On chest and plain abdominal roentgenogram, elevation of right hemidiaphragm and pleural reaction were shown in 54.9% and paralytic ileus in 77.5%, respectively.
8) On oral cholecystogram and intra-venous cholangiogram, gallbladder was non-visualized in 36.9%, and stones in bi liary tract in 26.2%.
9) The stones were located in the gallbladder(38.5%), common bile duct(33.7%), and both gallbladder and common bile duct(17.3%).
10) The number of stones was over 5 in 49.9% of calculous patients, and single in 23.1%.
11) The incidence of the associated disease was 13%.
12) The operative procedures were adopted according to the type of disease.
13) The indication for exploration of CBD were stones in CBD and its dilatation etc.
14) The post-oprative complications were wound infection, sepsis, and pneumonia etc.
15) The post-operative mortality rate was 4%. The causes of death were sepsis, hemorrh agic shock, and renal failure.
